Abstract Background: corporate wellness programmes have gained increasing attention as tools for promoting healthy behaviours in the workplace, yet systematic evidence on their diffusion in Italy is limited. Objectives: to describe the adoption of wellness practices among large companies in Emilia-Romagna (Northern Italy) and document companies’ perceived benefits. Design: cross-sectional study conducted in May-July 2024. Setting and participants: 32 companies among the 200 largest companies in Emilia-Romagna, identified through the regional Chamber of Commerce. Main outcome measures: the questionnaire covered four domains: active mobility, healthy eating, physical activity, and prevention initiatives, as well as companies’ perceived effects of wellness programmes. Results: each company was contacted both via email and via telephone; 150 were successfully reached and 32 completed the questionnaire (response rate: 21%). Among the participating companies (N. 32), 14 (44%) promoted active mobility initiatives, 26 (81%) offered healthy eating options, 6 (19%) provided in-house gyms, and 15 (47%) supported preventive health assessments. Among the 17 companies reporting positive organisational effects, 70% cited an improved working climate, 65% reported better employee health, 60% perceived greater corporate attractiveness, whereas only 22% reported perceived productivity gains. Nearly half of the companies (15; 47%) expressed willingness to expand wellness initiatives if supported by public policies or incentives. Conclusions: corporate wellness practices are widespread among large companies in Emilia-Romagna, though perceived productivity effects remain limited. The descriptive nature of the study, the modest sample size, and the potential non-response bias recommend caution, but findings highlight opportunities for policy support and low-cost wellness interventions.

Riassunto Introduzione: i programmi di corporate wellness hanno ricevuto crescente attenzione come strumenti per promuovere comportamenti salutari nei luoghi di lavoro; tuttavia, le evidenze sistematiche sulla loro diffusione in Italia sono limitate. Obiettivi: descrivere l’adozione di pratiche di wellness nelle grandi imprese dell’Emilia-Romagna e documentare i benefici percepiti dalle aziende. Disegno: indagine trasversale condotta tra maggio e luglio 2024. Setting e partecipanti: trentadue aziende fra le duecento più grandi aziende dell’Emilia-Romagna (identificate tramite la Camera di Commercio). Principali misure di outcome: il questionario ha coperto quattro domini: mobilità attiva, alimentazione sana, attività fisica e iniziative di prevenzione, oltre agli effetti percepiti dei programmi di wellness. Risultati: le imprese sono state contattate sia via email sia via telefono; 150 sono state raggiunte con successo e 32 hanno completato il questionario (tasso di risposta: 21%). Tra le imprese partecipanti (n. 32), 14 (44%) promuovevano iniziative di mobilità attiva, 26 (81%) offrivano opzioni di alimentazione sana, 6 (19%) mettevano a disposizione palestre interne e 15 (47%) supportavano valutazioni preventive di salute. Tra le 17 aziende che hanno segnalato effetti organizzativi positivi, il 70% ha citato un miglior clima lavorativo, il 65% ha riferito una migliore salute dei dipendenti e il 60% ha riscontrato una maggiore attrattività aziendale, mentre solo il 22% ha segnalato un incremento percepito della produttività. Quasi la metà delle imprese (15; 47%) dichiarava la disponibilità ad ampliare le iniziative di wellness in presenza di politiche pubbliche o incentivi. Conclusioni: le pratiche di corporate wellness risultano diffuse tra le grandi imprese dell’Emilia-Romagna, sebbene gli effetti sulla produttività percepiti siano limitati. La natura descrittiva dello studio, la dimensione campionaria contenuta e il potenziale bias da non risposta richiedono cautela nell’interpretazione; tuttavia, i risultati rilevano opportunità di supporto pubblico e di interventi di wellness a basso costo.
