A 69-year-old man with persistent breathlessness underwent a diagnostic workup that ruled out common possible causes, such as respiratory disease and heart failure with preserved ejection fraction. Clinical suspicion and specific imaging tests allowed the diagnosis of the rare platypnea-orthodeoxia syndrome (POS), secondary to a patent foramen ovale (PFO). In this context, the PFO tunnel and the intracardiac shunt are often large due to cardiac conditions favoring blood shunting and a distorted geometry, a consequence of concomitant thoracic anatomical alterations. Effective percutaneous PFO sealing was obtained using a device dedicated to atrial septal defect closure because of the large size and the aim of obtaining immediate relief from symptoms. Intraprocedural sizing was performed with techniques based on PFO tunnel stretching, to determine its actual size.
