Cavo-portal hemitransposition, by anastomizing the donor portal vein with recipient vena cava, was utilized in one case of liver transplantation in which the recipient portal, splenic and superior mesenteric veins were completely occluded. Graft function was normal and the postoperative course was complicated by ascites which resolved after one month and by two episodes of gastrointestinal bleeding due to hypertensive gastropathy treated successfully with transcutaneous splenic artery embolization. The patient is in good general condition and liver function tests are normal four months after liver transplantation.

Abstract
Cavo-portal hemitransposition, by anastomizing the donor portal vein with recipient vena cava, was utilized in one case of liver transplantation in which the recipient portal, splenic and superior mesenteric veins were completely occluded.
