Background: The benign nature of the surgical complications and the very low surgical mortality rate associated with laparo- scopic gastric banding procedures are among the key drivers of its exponential growth. Long-term weight loss results in the Italian series with the Lap-Band are presented, which further support the expanded utilization of this effective treatment modality. Methods: By the data-base of the Italian collaborative study Group for Lap-Band® (GILB), patients operated between January 1996 and December 1997 were allocated into 4 groups accord- ing to the BMI range at the time of Lap-Band® System position- ing: 30-39.9 (Group A), 40-49.9 (Group B), 50-59.9 (Group C), ≥ 60 (Group D) kg/m2. BMI and %EWL were considered in each group. Data were expressed as mean±standard deviation (SD), except as otherwise indicated. Results: After 5 and 7 years from Lap-Band® System surgery, 1060 and 197 out of 3,813 patients were eligible respectively. At 5 years follow-up patients were 733/1,060 (69%) in groups A, B, C and D, the mean BMI was: 27.7±5.3; 34.2±5.1; 39.9±6.9; 46.5±11.5 kg/m2, and the mean %EWL was 54.6±12.4; 54.1±10.9; 51.6±10.8; 59.1±18.6 respectively. At 7 years patients at follow up were 153/197 (77.6%) and in the same groups the mean BMI was 28.1±3.8, 31.7±5.5, 37.9±8.4, 43.9±12 kg/m2, and the mean %EWL was 59.4±11.6, 57.1±12, 56.6±10.4, 60.6±17.1. Conclusions: Mean 50% EWL is currently achievable by Lap- Band® surgery and maintained at 7 years follow-up in these 28 different Italian centers. Considering the absence of metabolic consequences and simplicity of these operations, Lap-Band® is a fundamental tool for the treatment of morbid obesity.
