Background: The higher incidence of complications and re- operation early in Lap-Band® experience has been considered by some the Achilles’ heel of this operation. The trend of the most common surgical complications in patients operated with the Lap-Band® over the years is analyzed. Methods: Complications of patients operated in 27 centers between January 1996 and June 2002 were analyzed. Mortality, gastric pouch dilation, tube-port failure, intra-gastric migration and revision operation were related to the year of Lap-Band® placement. Minimum patient follow-up is 1 year. Results: 3,355 patients were eligible. Mean mortality rate was 0.41% with a similar trend over the years, while complications and re-operation have decreased (Table). Conclusions: Factors aiding the progress of Lap-Band® surgery over the years are surgical technique improvement, system manufacturing evolution, and appropriate postoperative management.
