The purpose of this study was to investigate the in vitro growth of early (burst-forming units-megakaryocyte: BFU-meg) and late (colony-forming units-meg: CFU-meg) meg- akaryocyte progenitors in essential thrombocythemia (ET) patients in comparison with normal controls. We observed several alterations of the in vitro growth of both classes of megakaryocyte progenitors in response to recombinant cytokines that may play a relevant role in the pathogenesis of ET. The responsiveness to recombinant growth factors, in particular to rhIL-3, is increased in ET marrow. Moreover, although CFU-meg and BFU-meg are less sensitive to hTGF-Pl than are the normal cells, they are not completely insensitive, since inhibition is still observed at the higher doses of hTGF-P1. Such a disequilibrium in the sensitivity of megakaryocyte progenitors to either recombinant growth factors or inhibiting factors appears to be one of the most relevant in vitn, parameters of abnormal megakaryocytopoiesis in ET and suggests that the target cell of the neoplastic transformation in ET escapes the control of some physiological regulatory mechanisms.
