Chronic inflammatory demyelinating polyneuropathy (CIDP) is an immune disorder in which the role of cellular immunity seems presently predominant. A crucial event in cell-mediated inflammatory demyelination is the transendothelial migration of leukocytes regulated by adhesion molecules reciprocally expressed in endothelial and circulating cells. We studied by immunocytochemistry the expression of: ICAM-1, ELAM-1, VCAM-1, LECAM-1, LFA-1, Mac-1 , sLex and VLA-4 in sural nerve of CIDP and non-inflammatory neuropathies. Activated endothelial cells positive for ICAM-1, ELAM-1 and VCAM-1, and LFA-1 were present in all CIDP and in less than 25% of non-inflammatory neuropathies. LECAM-1, Mac-1, sLex, and VLA-4 positive cells were also present with significantly higher frequency in CIDP. The very high frequency of CAMs activation found in situ in CIDP strengthens the role of cell mediated immunity in the pathogenesis.
