On the basis of a short critical review of the most frequent techniques used to reconstruct columellar defects after cancer surgery the authors described a two-stage operation to cover defects of columella, nasal septum and nostril floor. Two naso-labial flaps based on a caudal pedicle are transposed and passed deep in the nostril into the vestibular cavity. The method is illustrated with a case report. The advantages of this technique in comparison with others are emphasized.
