Objectives- Carpal tunnel syndrome (CTS) is one of the most common pathologies of the peripheral nervous system. The aim of this study is to collect, through a narrative review of the literature, the scientific evidence on CTS, particularly regarding the accuracy of diagnostic tests and the validity of treatments employed in clinical practice. Materials and methods – A research has been carried out between may 2008 and december 2009 in the main biomedic electronic databases (Pubmed, PEDro, EMBASE, CINAHL, Cochrane Library) and in the Libraries of the Istituti Ortopedici Rizzoli – Bologna (Italy) and of the Azienda Ospedaliero-Universitaria - Bologna. The following key words have been used: “Carpal tunnel syndrome”, “Physical examination”, “Nerve conduction testing”, “Median neuropathy”, “Tension test”, “Diagnostic testing”, with no limits of date and in English, Spanish, Portuguese, French and Italian languages. Results – The rates of reliability and diagnostic accuracy of various tests commonly used in CTS appear conflicting. To make a correct diagnosis, it is better to use a cluster of tests, rather than a single test. Conservative treatment is indicated in initial and episodic forms of CTS. Regarding the efficacy of neurodynamic techniques, only few scientific studies have been found and their results are questionable. Discussion and conclusions – The use of "clinical prediction rules" may be the most interesting perspective for the diagnosis of CTS. More research are needed to evaluate the effectiveness of non-surgical treatment, to compare variuos conservative treatments, and to identify which therapy is the most effective in different forms and phases of CTS.

Obiettivi - La sindrome del tunnel carpale (STC) è una delle più diffuse neuropatie periferiche. Scopo di questo studio è di raccogliere, attraverso una revisione narrativa della letteratura, le evidenze scientifiche su tale condizione patologica, analizzando l’accuratezza diagnostica dei principali test e la validità dei trattamenti utilizzati nella pratica clinica. Materiali e metodi - Tra maggio 2008 e dicembre 2009 è stata effettuata una ricerca nelle principali banche dati biomediche (Pubmed, PEDro, EMBASE, CINAHL, Cochrane Library) e negli archivi cartacei della Biblioteca degli Istituti Ortopedici Rizzoli di Bologna e della Biblioteca Centralizzata dell’Azienda Ospedaliero-Universitaria di Bologna. Sono state utilizzate le seguenti parole chiave: “Carpal tunnel syndrome”, “Physical examination”, “Nerve conduction testing”, “Median nerve neuropathy”, “Tension test”, “Diagnostic testing”, senza limiti di data e nelle lingue inglese, spagnola, portoghese, francese e italiana. Risultati – I valori di affidabilità e di accuratezza diagnostica di svariati test utilizzati comunemente nella STC sono apparsi discordanti. Per una corretta diagnosi, è preferibile l’utilizzo di cluster di test, piuttosto che di test singoli. Il trattamento conservativo è indicato nelle forme iniziali e transitorie. Per quanto riguarda l’utilizzo delle tecniche di neurodinamica, sono stati reperiti pochi studi e dall’esito conflittuale. Discussione e conclusioni – L’utilizzo di “clinical prediction rules” nella STC apre prospettive interessanti sul piano diagnostico. Sono necessari ulteriori studi clinici sull’efficacia del trattamento conservativo, per individuare quale terapia sia più efficace nelle diverse forme e fasi cliniche che questa sindrome presenta.
