Introduction Neck pain is one of the most common musculoskeletal disorders and it implicates several economical and social consequences. The efficacy of the education strategies of Cognitive Behavioural Therapy (CBT) has not been clearly determined yet. This systematic review aimed at finding the evidence of the efficacy of this therapeutic method in non-specific neck pain. Methods Bibliographic research was carried out from March 17th, 2008 to September 30th, 2009. The detected studies were examined and classified according to specific inclusion and exclusion criteria. Controlled trials concerning the effectiveness of CBT versus conventional trreatments were evaluated by means of JADAD Scale and PEDro Scale. Results Fifty-eight studies were found, eight of which (two reviews and six clinical trials) met the inclusion and exclusion criteria. Clinical trials quality was good in three studies, but scarce in the other three. The findings of included trials did not show significant differences between conventional and multidisciplinary therapy with respect to neck pain. The two evaluated systematic reviews confirmed this statement. Conclusion This systematic review does not allow to assert that CBT leads to better results than conventional therapies. As reliable results lack, specific clinical trials are needed in order to study further CBT techniques specifically with regard to neck pain.

Introduzione Nell’ambito dei disturbi muscoloscheletrici la cervicalgia è una delle problematiche più comuni, e presenta notevoli implicazioni sul piano economico e sociale. L’efficacia delle strategie educative della Terapia Cognitivo-Comportamentale (TCC) non è ancora definita con precisione. Questa revisione della letteratura ha lo scopo di indagare quali siano le prove di efficacia di tale strumento terapeutico nella cervicalgia aspecifica e quale sia la sua efficacia in riabilitazione. Metodi La ricerca bibliografica è stata effettuata nel periodo compreso tra il 17 marzo 2008 e il 30 settembre 2009. Gli studi reperiti sono stati analizzati e catalogati secondo criteri predefiniti di inclusione ed esclusione. Gli studi controllati riguardanti il confronto tra TCC e trattamenti standard sono stati valutati con la JADAD Scale e la PEDro Scale. Risultati Sono stati reperiti 58 studi, di cui solo 8 (2 revisioni e 6 trials clinici) hanno soddisfatto i criteri di inclusione ed esclusione. La qualità dei clinical trials è risultata buona per 3 studi, ma scarsa per i rimanenti 3. Dai risultati dei 6 studi clinici inclusi, non emergono differenze sostanziali rispetto all’outcome del dolore cervicale, confrontando la terapia usuale e quella multidisciplinare. Le due revisioni sistematiche analizzate confermano l’impossibilità di avanzare conclusioni in merito. Conclusioni Questa revisione della letteratura non consente di affermare che la TCC sia superiore al trattamento fisioterapico convenzionale. Non potendo considerare soddisfacenti i risultati ottenuti, è impellente la necessità di sperimentazioni scientifiche mirate allo studio delle tecniche cognitivo-comportamentali nell’ambito specifico della cervicalgia.
