Study Design: Systematic review of the literature. Introduction: The multifidus is considered one of the most important muscles for the lumbar spine stabilization and low back pain seems related to its dysfunction. The purpose of this review is to identify the most effective and clinically evidenced exercises to improve the tropism and the recruitment of the multifidus muscle. Methods: A literature review was conducted by two independent reviewers in Medline, PEDro, Cochrane, DARE, Scopus, Cinhal, PubMed, Tripadatabase databases. Studies on morphological and functional multifidus changes detected by instrumental data were selected. Limits were related to the studies in Italian or English languages, with available full text, on human adults suffering from specific or non-specific low back pain and without any date limitation. Results: From the 1854 articles derived from primary search, only two papers responded to the research question. They showed that the cross-sectional area and the thickness of the multifidus muscle can increase through some exercises addressed to the activation of this muscle, progressing from the motor control to a workout static and dynamic loads. We did not found no study related to a single exercise nor study using as an outcome measure the electromyographic recording. Conclusions: The results of this systematic review suggest that it is possible to improve the morphological and/or functional characteristics of the multifidus muscle through a series of specific and structured exercises. These findings may help physical therapists during their clinical practice to select the best exercises for the recruitment of the multifidus muscle.

Introduzione - Il multifido è considerato tra i muscoli più importanti per la stabilizzazione lombare e la sua disfunzione sembra correlarsi al dolore lombare. Scopo di questa revisione è individuare gli esercizi più efficaci per migliorare il trofismo e il reclutamento del multifido, in base all’evidenza clinica. Metodi - Una revisione della letteratura è stata svolta da due revisori indipendenti nelle banche dati Medline, PEDro, Cochrane, DARE, Scopus, Cinhal, PubMed, Tripadatabase. Sono stati inclusi studi in lingua italiana o inglese, su esseri umani adulti, senza limiti di data e con full text disponibile. Gli studi inclusi dovevano riguardare la possibilità di variare le caratteristiche morfologico/funzionali del muscolo multifido attraverso esercizi specifici, rilevate attraverso dati strumentali, in soggetti con lombalgia specifica o aspecifica. Risultati - Degli iniziali 1854 articoli ricavati dalla ricerca primaria, soltanto due lavori hanno risposto al quesito di ricerca. Essi hanno evidenziato che l’area trasversa e lo spessore del multifido possono incrementare attraverso una batteria di esercizi indirizzati all’attivazione di questo muscolo, progredendo dal controllo motorio verso un allenamento statico e dinamico a carichi crescenti. Non è stato reperito alcuno studio inerente ad un singolo esercizio o che utilizzasse come misura di outcome la registrazione elettromiografica. Conclusioni – I risultati di questa revisione sistematica sembrano indicare la possibilità di migliorare le caratteristiche morfologico/funzionali del muscolo multifido attraverso una serie di esercizi specifici e strutturati. Questi risultati possono aiutare il fisioterapista nella pratica clinica, per la scelta dei migliori esercizi per il muscolo multifido.
