It is not possible to stage MSGCs of the oral cavity according to the staging proposed by the 8th edition of the TNM classification, because errors may arise in T staging and consequently in treatment. Further studies are needed to clarify the role of DOI in MSGCs, before to include it in routine TNM staging. In our opinion and experience, the concept of DOI is not applicable when the basement membrane is not infiltrated by the tumor. It would therefore be appropriate to propose an ad hoc staging for MSGCs of the oral cavity, in order to take into account not the DOI but the potential routes of spread of the tumor. In this way, treatment would be tailored to the type of tumor invasion and thus to the patient.
