Extramammary Paget disease (EMPD) is an uncommon intraepithelial adenocarcinoma, with prognosis largely dependent on its diagnosis as either primary or secondary EMPD. Surgery may not always be viable, particularly in sensitive areas or in patients with impaired aggregation/coagulation mechanisms. This report describes a case of recalcitrant primary scrotal EMPD associated with prostate cancer (PCa) and May-Hegglin anomaly successfully treated with one session of rhenium brachytherapy.
