This study adds important information on the possibility of DBR in virologically suppressed multidrug-resistant PWH, although the impact of this therapeutic approach on the risk of failure is still unclear. In our study, the only identified predictor of VF was a low baseline CD4+/CD8+ ratio. Given the urgent need for regimen simplification in the fragile population with 4DR HIV and the availability of new options, the support of an expert in HIV resistance is advised in this situation and new tools to predict the virological safety of reducing drug burden should be investigated.
