Gamma-hydroxybutyrate (GHB) is an approved drug for the treatment of narcolepsy/cataplexy and a compound diverged for recreational purposes. GHB exerts a euphoric but also sedative effect on the central nervous system and has been linked to several intoxications and a number of death cases. It has been also used as a performance enhancing drug, especially in the setting of bodybuilding, as well as to enhance communicative skills and sexual performances. Its use among apprehended drivers, given its development as an anesthetic agent, poses several challenges to traffic safety. The present chapter discusses the recent research in human models and in epidemiological/real forensic case studies regarding the effect of GHB on psychomotor performances, with particular reference to those involved in driving abilities.
