In order to develop innovative solutions to materialize the "continuum of care" vision, an ecosystem of interactive devices based on heterogeneous technologies is required. This paper describes an architecture to ensure the interoperability of devices and services. A middleware to support this architecture was developed in two European ARTEMIS Projects: CHIRON and SOFIA.
