Free fat grafts in plastic surgery and in regenerative medicine are extremely promising. Their use in breast reconstruction and in particular after radiotherapy is radically changing the approach to the problem. The chapter presents a wide review of the literature and the experience of the authors about the salvage of pre-exposed prostheses and expanders, aesthetic and functional improvements in reconstructed irradiated breasts and the total breast reconstruction with fat grafts only.
