This paper examines The Pitt (HBO Max, 2025–) in relation to ER (NBC, 1994–2009), tracing a genealogy of the American medical drama between continuity and renewal. While The Pitt retains key elements of ER, including ensemble storytelling, the intensity of emergency medicine, and a focus on ethical dilemmas, it adapts the genre to the narrative and social conditions of the post-network era. Its highly serialized structure, compressed fifteen-hour timeframe, and use of cliffhangers and flashbacks distinguish it from the episodic model of earlier medical dramas. The series also transforms the hospital into a space of explicit social and political critique, addressing healthcare inequalities, labor precarity, multiculturalism, post-Covid trauma, and gun violence. The paper argues that The Pitt represents a new phase of the medical drama, in which the hospital functions not only as a mirror of society but also as a site for examining contemporary institutional and systemic crises.
