In this chapter, three surgeons from various regions of the world review a case of patellofemoral (PF) instability in the adult female with recurrent instability, the chondrosis of patella and trochlea. The case was 40 years old female who underwent medial patellofemoral ligament (MPFL) reconstruction for recurrent patellar instability on her left knee. Her patellar recurrent instability was improved due to MPFL reconstruction; however, the patient still had symptomatic bipolar full-thickness chondral lesions, and slight maltracking of the patella remained. Thus, she underwent anteromedial tibial tubercle transfer and autologous chondrocyte implantation for bipolar chondral lesions of the PF joint. At 6 years after the surgery, she was doing well without any symptoms on her left knee. This case-based discussion will include an outline of factors that influence surgical decision making, recommended treatment approaches.
