The isolated right superior caval vein draining into the left atrium is an extremely rare systemic venous malformation. Pediatricians must be aware of it especially in apparently healthy children with mild cyanosis in absence of respiratory symptoms and signs of cardiac malformation. Echocardiography, angio-computed tomography and angio- MRI can confirm the suspicion.
